Gait Analysis and Biomechanics in Podiatry
What is Gait?
Gait is the way a person walks or runs.
It includes:
- How your feet strike the ground
- How your legs move and support your body
- How your hips and spine absorb and transfer movement
When the gait pattern is abnormal, it can lead to:
- Foot pain
- Knee or hip pain
- Back pain
- Reduced walking efficiency
Why Gait Matters
Your feet are the base of your body.
If something is not aligned at the feet, the rest of the body compensates, which may cause pain elsewhere.
Think of the feet like the foundation of a building.
If the foundation tilts, the whole structure adjusts and becomes stressed.
What is Gait Analysis?
Gait analysis is the process of observing and measuring how you walk to identify:
- Pressure distribution
- Stride length
- Foot alignment
- Muscle balance
- Joint motion
This helps the podiatrist understand why pain, discomfort, or imbalance is occurring.
| Step | What Happens |
| 1. Medical & activity history | Discuss symptoms, sports, footwear, previous injuries |
| 2. Visual observation | Watching walking and standing posture from different angles |
| 3. Pressure & motion testing | Foot pressure mats or video slow-motion analysis |
| 4. Joint and muscle testing | Checking strength, flexibility, and limb alignment |
| 5. Treatment planning | Footwear changes, therapy, orthotics, exercise program |
Common Gait Patterns Identified
| Gait Pattern | Description | Possible Outcomes |
| Over-pronation | Foot rolls inward too much | Heel pain, shin splints, knee pain |
| Supination (Under-pronation) | Foot rolls outward too much | Ankle sprains, hip pain |
| Short stride or limping gait | Pain or muscle imbalance limits motion | Fatigue, stiffness, discomfort |
| Toe-walking (common in children) | Walking on toes instead of heel-to-toe | Tight muscles, balance issues |
Role of Biomechanics
Biomechanics looks at how bones, muscles, and joints work together during movement.
A podiatrist evaluates:
- How body weight transfers across the legs and into the feet
- Whether joints move smoothly
- Whether muscles are working too hard or too little
- How footwear affects walking
This ensures the feet are supporting the body correctly.
Treatment Based on Gait & Biomechanical Findings
| Treatment | Purpose |
| Custom Orthotics (insoles) | Improve alignment and reduce strain |
| Footwear recommendations | Choosing shoes that support the foot shape |
| Strengthening exercises | Improve muscle support and stability |
| Stretching programs | Reduce tightness and improve motion |
| Activity modification | Adjusting movement to reduce pain and overload |
Who Benefits from Gait Analysis?
- People with foot, ankle, knee, hip, or back pain
- Runners, athletes, or active individuals
- Children with flat feet, in-toeing, or toe walking
- Older adults experiencing stability or balance concerns
- Anyone recovering from injury or surgery
